1. The big problem is how Medicare/Medicaid drive discounts as opposed to actual costs.

   Nuke the discount plan (where Medicare/Medicaid get the largest discounts by law) and move to an actual cost plan where people are aware of the costs of a procedure and you will immediately decrease the costs of healthcare.
