It goes back over a century. And ObamaCare didn’t address it, and just made it worse.

The price of a good or service will always try to rise to the base subsidy plus whatever someone is willing and able to pay.
With Obamacare, there were a lot of things that providers were allowed to charge for that would be covered by insurance companies that used to not be. Once those dreaded MBAs found these little gold nuggets, providers started milking them. One example, new patient fees.
The corrupt system incentivizes inventing new charges. They jack up prices and give you a “discount” to make you feel like you got a deal rather than robbed. They charge you a monthly fee but they also know what the typical person is willing and able to pay on top of it and charge that each visit. They also use these additional out of pocket costs as a disincentive to going to the dr, so you pay hundreds and hundreds of dollars every month but then don’t have the extra cash to get medical care. Enjoy your one free visit every year where you are limited in what you can discuss.
Providers are all in on the corruption and shenanigans too as they are forced to cover Democrat’s ideological demands. The Unhoused Militia isn’t getting free healthcare, we pay for it through accounting tricks, over and mis charges, and straight up subsidization.
The system is designed to corrupt everyone involved in it.
Kinda of mirrors the original sin of public education…
