“Incorrectly installed”. My mind boggles. Did they install it upside down or back to front? Forget to safety the nuts holding it in place?
Clearly not enough ground testing was done before committing to flight.
Presumably it means that they accidentally forgot to check the “no crash” box on the installer wizard.
